Once we take possession of the birds they become the sole property of the For Birds Foundation and you release all ownership rights to us and you give up all future rights to reclaim, regain custody of, or adopt your surrendered birds from us. By signing below, you acknowledge and agree to these statements. No payment will be made for your surrendered birds. Are you able to donate funds to For Birds Foundation to cover some of the health exam costs? Are you surrendering more than one bird? If yes, how many? What is the birds name? What species is the bird? Do you know if the bird is male or female and if so, was this verified by a DNA test? Why are you surrendering your bird or birds? Please be very specific as to the reasons. This questionnaire is confidential and there is no judgement. Our only care is for the welfare of the bird and if you feel you need to surrender it to us we accept your reasons. What is the name of the birds current veterinarian? This document will be printed out and signed by you in person at the time of surrender.
